Pure Obsessional OCD — commonly called "Pure O" — refers to OCD in which a person primarily experiences intrusive, distressing obsessive thoughts without obvious outward compulsive rituals. It may take on the form of many OCD subtypes such as Harm OCD, HOCD, ROCD, or Scrupulosity OCD. What distinguishes Pure O is that the compulsions are primarily mental rather than behavioral.
Despite its name, Pure O is not truly "pure" — compulsions do exist, but they happen internally. These mental compulsions include mentally reviewing events to seek certainty, neutralizing unwanted thoughts with "good" thoughts, mentally arguing against the intrusive thought, and seeking reassurance from others or online. These hidden rituals can be just as time-consuming and exhausting as physical compulsions.

Because there are no obvious rituals, Pure O is frequently misdiagnosed as generalized anxiety, depression, or even psychosis. Sufferers often feel intense shame about their thoughts and are reluctant to seek help. Without a specialist who understands OCD, treatment can be delayed for years.
Pure O responds very well to specialized OCD treatment. Cognitive Behavioral Therapy (CBT) with Exposure and Response Prevention (ERP) and Inference-Based CBT (I-CBT) are both highly effective. The key is learning to resist engaging in mental compulsions — which reinforces obsessional doubt — and instead practice sitting with uncertainty.
